1. Scope and goals

The International Consortium for Therapeutic Drug Monitoring (spECTRUM) is a global collaboration comprising 53 scientists—including 49 gastroenterologists, 1 dermatologist, 1 rheumatologist, 1 clinical pharmacist, and 1 pharmacometrics specialist—from 23 countries across five continents. The consortium is dedicated to evaluating the clinical utility of therapeutic drug monitoring (TDM) of biologics in inflammatory bowel disease (IBD), with the overarching aim of advancing personalized medicine and improving patient care.

Scope:

  • To define the clinical impact and utility of TDM for biologic therapies in IBD.

Goals:

  1. Develop large-scale clinical databases and conduct both retrospective and prospective studies to address key knowledge gaps.
  2. Initiate randomized controlled clinical trials, contingent on funding availability.
  3. Conduct surveys of patients and healthcare providers to assess the adoption of TDM and identify barriers to its implementation in clinical practice.
  4. Translate insights from TDM in IBD to optimize biologic therapy in other immune-mediated inflammatory disorders, including rheumatoid arthritis and psoriasis.
  5. Incorporate emerging innovations, such as point-of-care devices and pharmacokinetic modeling, to enhance the practical application of TDM in clinical care.

2. Members
The consortium consists of 53 members representing 23 countries across five continents, encompassing a multidisciplinary team of specialists focused on advancing TDM in IBD and related conditions.
