The Fellowship in Emergency Medicine Administration and Operations prepares residency-trained emergency physicians to become skilled leaders in the operational, financial, and strategic management of the emergency department. Fellows gain hands-on experience with the full scope of ED administration — from patient flow and crowding mitigation to staffing models, physician recruitment, and institutional governance — while continuing to practice clinically as attending physicians. The program is designed for graduates of an ACGME-accredited emergency medicine residency who are seeking a structured pathway into departmental or institutional leadership roles.
Alongside the didactic curriculum covering a wide range of topics from throughput to budgeting, fellows are expected to develop and advance an independent research project, with regular check-ins and structured research updates built into the weekly schedule. This combination of operational coursework, applied skills training, and scholarly output prepares fellows not only to manage the day-to-day operations of an emergency department, but to lead system-level initiatives — staffing redesigns, financial planning, quality programs, and workforce wellness efforts — that shape the department's long-term performance and the careers of the physicians within it.
Fellows can choose to do either a 1- or 2-year fellowship depending on whether there is interest in pursuing an advanced degree. Popular advanced degree options include Master of Clinical Service Operations at Harvard Medical School or a Master of Public Health at Harvard TH Chan School of Public Health. Fellows will work clinically as attendings at BIDMC and one of the HMFP community affiliate hospitals with a shift reduction to facilitate learning, project work and, if desired, advanced degree coursework.

Curriculum Highlights
The curriculum is built around a weekly didactic series that rotates through core lectures, applied skills workshops, and dedicated research time. Core sessions cover the operational backbone of ED leadership: crowding and boarding, staffing and advanced practice provider utilization, recruitment and compensation models, institutional finance and revenue cycle, observation unit development and billing, clinical pathways, quality assurance and process improvement, and patient experience. These are paired with skills-focused workshops on negotiation, budgeting and P&L analysis, change management, project management, queueing theory, and data analysis in Excel — practical competencies fellows apply directly to real departmental problems. A monthly case-study rotation grounds this material in real-world scenarios, such as staffing a new ED integration, building a QA program, managing high utilizers, or responding to shifts in patient volume and budget.
