Currently there are two other clinical demonstration projects from the VA Boston Healthcare System

• Geriatrics in Primary Care Clinical Demonstration Project
  - Dr. Peter Engel - 
  Coinciding with the initiation of Patient Aligned Care Teams (PACT), the Geriatrics in Primary Care program seeks to streamline clinical care for older veterans with multiple cognitive and functional deficits by performing evaluations during the same visit as primary care and immediate communication with the PACT team.

• PILL Clinic Clinical Demonstration Project
  - Dr. James Rudolph - 
  The longstanding, successful PILL Clinic model has been adapted to capture patients upon transition from inpatient to home. The initial results found a high prevalence of errors in discharge instructions to cognitively impaired patients and the new PILL model accomplishes this by electronically targeting patients in performing telephone reconciliation.
