Program Overview

The breast imaging fellowship at Beth Israel Deaconess Medical Center (BIDMC) is a one-year fellowship with extensive experience in diagnostic and interventional multimodality breast imaging and now multidisciplinary breast oncology care, through the new collaboration with Dana-Farber Cancer Institute (DFCI). Upon completion of this fellowship, candidates will be competent to pursue any career in breast imaging, whether it be at an academic center or a private practice as subspecialty consultant.

The newly combined breast fellowship program will be split between the main BIDMC Boston longwood campus and the two DFCI locations in Boston Longwood and Chestnut Hill.  At the main BIDMC Boston Longwood outpatient campus, fellows will be a part of a high-volume center where over 10,000 screening mammograms, 8,500 diagnostic mammograms, 4,500 ultrasounds and 1,000 MRIs are performed each year.  Fellows will be involved in interpretation of digital tomosynthesis screening and diagnostic mammograms and participate in hand-on ultrasound scanning.  Fellows will also participate in interpretation of high-volume screening and diagnostic breast MRI at about 80 cases per week. BIDMC has a robust contrast-enhancement mammography program where fellows will participate in interpretation and CEM interventions.

At BIDMC, there is an active interventional service with over 40 breast procedures per week. Fellows have the responsibility of performing challenging multimodality non-wire localizations, US and stereotactic-guided core biopsies as well as MRI biopsies each week.

Breast fellows will also spend time at Dana Farber Cancer Institute working in a high-risk diagnostic imaging clinic and providing imaging consultation services to surgeons, medical oncologists and radiation oncologists in a multidisciplinary clinic setting for patients with a new diagnosis of breast cancer.

In addition to the rich clinical experience, fellows will participate in teaching and clinical research, needle biopsy training in our simulation center, journal club, weekly multidisciplinary conferences, and radiology-pathology correlation conference.

This fellowship program includes 48 weeks of dedicated breast imaging training and 4 weeks of elective time. In the past, fellows used elective time for clinical training in other radiology divisions, extended research time, or other non-clinical interests. In addition, fellows rotate through breast pathology, radiation therapy, nuclear medicine and the breast care center to get a complete overview of what is involved in caring for and managing patients with breast issues.

Academic time will be given for research and/or QI/QA projects. The fellow will work with a faculty mentor to design a research project(s), obtain IRB approval, collect and analyze data and prepare manuscripts for presentation and/or publication.

Six practice sites allow exposure to MQSA and ACR inspections throughout the year and our QA program provides opportunities for focused learning around difficult/complex cases and system improvements for patient care.

Program Goals

  1. To provide an organized, comprehensive, and highly supervised educational experience in breast imaging so that the fellow can function as an independent breast imaging consultant and practitioner in academic or private practice settings
  2. To train fellows to screen average-risk and high-risk populations for breast cancer using digital tomosynthesis mammograms, contrast-enhanced mammography (CEM), and high-risk breast MRI
  3. To train fellows in the selection, performance, and interpretation of:
    • diagnostic breast imaging studies in all modalities
    • interventional procedures along with an understanding of radiologic-pathologic review for concordance of pathology findings
  4. To provide sufficient experience to advise referring clinicians on the most expeditious and efficient imaging techniques to answer clinical and imaging breast problems
  5. To encourage and provide opportunities and skills for research in the field of breast imaging
  6. To train fellows in quality control, quality assurance, and peer review for breast imaging to comply with all state and federal regulations.
  7. To provide fellows with the skills in diagnostic imaging interpretation of patients with a new diagnosis of breast cancer.
  8. To train fellows to become a valued and integral member of the multidisciplinary breast oncology care team

Curriculum

We offer a comprehensive curriculum that spans all aspects of breast imaging with a robust didactic curriculum as well as ample hands-on experience. Some highlights are included below. Please contact us if you would like additional detailed information about the curriculum.

Rotations

Fellows typically have week-long rotations in various aspects of breast imaging that include screening mammography, diagnostic mammography and ultrasound, breast procedures (including ultrasound-guided core biopsies, stereotactic core biopsies, MRI core biopsies, and localizations) and breast MRI. Our high volume provides fellows with ample clinical exposure.  Fellow rotations will be between the BIDMC main breast center and the DFCI sites, allowing exposure to both average and high-risk patient populations and the management of patient’s with newly diagnosed breast cancer.  As part of the DFCI experience, fellows will also provide imaging consultation to multidisciplinary breast oncology colleagues.

Research

High-quality research has always been a priority of the Breast Imaging division of BIDMC. The primary work of our group has evolved to focus on new technology, value-based healthcare, and education. To promote further advancement in research related to breast imaging, we have partnered with members of VIR, breast pathology, breast surgery, and primary care on projects. Our program provides many opportunities for trainees to become involved in scholarly projects. Each fellow will work with a faculty mentor to design a research project(s), obtain IRB approval, collect and analyze data, and prepare manuscripts for presentation and/or publication. Academic time will be given for research and QI projects.

Conferences

Dedicated breast imaging lectures covering the essential topics of breast imaging will be given by BIDMC/DFCI faculty weekly to the fellows. Additionally, fellows will participate in the Society of Breast Imaging’s Breast Imaging Fellowship Consortium (BIFC) weekly didactics and quarterly journal clubs.  Monthly conferences also include fellow-lead interesting case conferences, journal clubs and research meetings.  Fellows will also participate in multiple multidisciplinary conferences including weekly Breast Radiology Pathology Correlation Conference and Multidisciplinary Breast Cancer Tumor Board. Fellows also participate in quarterly multidisciplinary ‘classics’ journal club with the pathology and surgery departments where we review key articles in breast literature.

Elective Time

Each fellow is granted four weeks of elective time, which may include clinical time in other radiology divisions, extended research time, or time to pursue other non-clinical interests such as leadership or quality assurance.
